Registration forms will be available in Cedarburg Schools and at the Recreation Department starting on  February 1, 2012. This program is open to children living in the Cedarburg School District currently in K through 8th grade. Child must be 5 prior to June 11, 2012 to participate.

All practices and games will be during the weekday evenings.

Game Days & Fields Used

PRACTICES AND GAMES:  Your child could have practice on any day of the week during the month of May dependent upon field availability and coach’s schedule.  Games will be played in the evenings Monday through Thursday from June to early August.

HOW TO REGISTER:  Complete the registration form below, include correct registration fee and return to the Cedarburg Parks and Recreation Department.  Office hours – Monday through Friday 8:30 to 11:30 a.m. & 12:30 to 4:30 p.m.  Mail – W63 N645 Washington Ave. PO Box 49 Cedarburg WI  53012 (lost or delayed mail is not the responsibility of the Parks and Recreation Department).  Drop Box – located at the south entrance of City Hall (please place in an envelope marked Parks and Recreation). You can also register online this year!

REGISTRATION BEGINS: WEDNESDAY, FEBRUARY 1, 2011

DEADLINE:  The registration deadline is FRIDAY, MARCH 23, 2012. Programs have limited enrollment and may fill prior to the deadline date!!!!!!!!   Any registrations received after the deadline will be placed on a waiting list if program is already filled. 

COACHING:  Help is needed with coaching all teams.  If you are available and/or willing please indicate so on the registration form below.  You will be asked to coach your child’s team.  No experience is necessary just enthusiasm and a desire to work with youth.  Your help is appreciated!   Please NO COACHING OR CO-COACHING REQUESTS!; all teams need coaches.

*Coaches will not be contacted for a reminder regarding meetings – if you indicate you will coach on the registration form, please attend the appropriate meeting.  If you are unable to attend, please contact the Parks and Recreation Department at 375-7611.

PLAYER REQUESTS:  In order for any request to be considered please follow these guidelines: 1) Print the name of the player your child requests on the registration form and that child must put your child’s name on his/ her registration form.  2) Only put one request on your registration form.  3) Coaches cannot be requested by players.  If any of these guidelines are not met your request may not be considered.  Please remember this is a request and no guarantees can be made.
